📍 Location & Trade
We now cover all 50 states — including California, Washington, Oregon, and all other states with their own OSHA programs.

🏛️ State-Specific OSHA Standards

Your state has additional safety requirements
These questions help us include the right state-specific sections in your program.
People & Emergency Contacts
Who handles safety on your crew? Most small shops finish this in under 2 minutes.
SAFETY OFFICER
This is the person workers go to with safety questions. For most small crews, it's the owner.
Please enter the safety officer name.
A cell or direct number — not the main office line, if possible.
Please enter a valid 10-digit phone number.
A foreman or senior crew member who can handle safety decisions when the main person is out.
Backup must be different from the safety officer.
FIRST AID
OSHA requires at least one trained first-aid responder on each job site. Don't worry if the answer is no — your program will include what you need to get this set up.
Please select an answer.
The person with a current First Aid/CPR certification.
Please enter the first aid responder name.
The date on their card. An approximate date is fine.
This certification may be expired. Standard First Aid/CPR cards are valid for 2 years.
Please enter the certification date.
Usually 2 years from certification. Check the card if you're not sure.
This certification appears to be expired.
RESCUE PERSONNEL
OSHA requires a rescue plan before anyone works in a harness. This can be the same person as your safety officer. If you have more than one, separate names with commas.
The date of their most recent rescue training.
Annual refresher training is recommended.
Important if you run two or more crews at the same time.
EMERGENCY CONTACTS
This name goes on the emergency contact card every worker carries. Usually the owner or safety officer.
Please enter the emergency contact name.
This goes on every worker's emergency contact card.
Please enter a valid 10-digit phone number.
If someone gets hurt on an evening or weekend job. Leave blank if same as above.
We put this in your Emergency Action Plan. If you work across different areas, use the one nearest your main shop or yard.
Leave blank to default to 911.
SITE DETAILS
This goes on your emergency contact cards and your Emergency Action Plan.
Street address — goes on emergency cards so crews can navigate there.
Your meeting spot in an emergency — like "front parking lot by the dumpster" or "across the street at the gas station." If it changes per site, describe your default.
SDS binders, crew truck glove box, digital app — wherever your crew can access chemical safety info on the job site.
This helps us size your emergency and rescue coverage. Most small shops are 1-2.
Insurance, Licensing & Equipment
These details go straight into your safety program — no brackets or blanks.
WORKERS' COMPENSATION
Check your Certificate of Insurance (COI) if you're not sure — your carrier name is at the top.
Please enter your workers' comp carrier name.
This is printed on your Certificate of Insurance and your policy declarations page. It usually looks like a mix of letters and numbers (e.g., WC-2026-55431).
Please enter your workers' comp policy number.
Usually January 1 or whenever you last renewed. Check your COI for the "Effective Date."
Please enter the coverage start date.
Usually 12 months after the start date. Check the "Expiration Date" on your COI.
Please enter the coverage end date.
Your EMR measures your company's injury history compared to similar companies. A score of 1.0 is average. Below 1.0 means fewer injuries than average. Your insurance carrier or broker can tell you your current EMR.
In Texas, workers' comp is optional. If you carry it, you're a "subscriber." If you don't, you're a "non-subscriber." Most small contractors in Texas do carry workers' comp.
Please select your workers' comp status.
Texas law requires you to post a notice telling employees whether you carry workers' comp. Most small contractors use DWC Notice 6.
Please select the DWC notice form.
CONTRACTOR LICENSING
The license classification on your state contractor license. Examples: "General Building Contractor (B)", "Electrical Contractor (C-10)". Select "No state license required" if your state doesn't require one.
Please enter your license type or "No state license required."
The number on your state-issued contractor license. If your state doesn't require a license, you can skip this.
The qualifying individual named on the license. For many small contractors, this is the owner.
Please enter the license holder name.
MASTER ELECTRICIAN
Texas TDLR requires a licensed Master Electrician to supervise electrical work. This may be the owner, an employee, or a contracted license holder.
Please enter the Master Electrician name.
The TDLR-issued license number. Format is usually TDLR-ME-XXXXXX.
Please enter the Master Electrician license number.
TDLR electrical licenses renew annually. Check the expiration date on the license card.
This license may be expired. Please verify.
LOCKOUT/TAGOUT EQUIPMENT
Select all that apply. Your safety program will include specific lockout procedures for each equipment type you select.
Please select at least one equipment type.
List equipment types not covered above. Example: "Conveyor belts, industrial ovens."
Please describe the other equipment.
OSHA requires locks and tags to be accessible to authorized employees. Common locations: "Main tool trailer, wall-mounted station by entrance" or "Gang box on each crew truck."
Please enter where you keep your LOTO equipment.
COMPANY CONTACT
This goes on the cover page and employee contact sections of your safety program.
Please enter a valid email address.
If you have one, we'll include it on your program's cover page. No website? No problem.
